Gap Assistance Pharmacy Program for Seniors (GAPS)
South Carolina Department of Health and Human Services
The GAPS (Gap Assistance Pharmacy Program for Seniors) form is for SC residents
only. It is for seniors 65 or older who are Medicare eligible and have a gross
household income below 200 % of the Federal Poverty Level and who need help
"filling the gap" with their Medicare prescription drug plan.
IMPORTANT!!! If you are a senior 65 or older, you can submit your GAPS Application online! You must register (on this site) to submit your GAPS application electronically.
All senior residents of South Carolina who are 65 or older may print the form and mail it to the SC Department of Health and Human Services for processing.

In order to save your GAPS application and be able to go back to it at a later time, you must Register.

IMPORTANT! For electronically filed GAPS applications
The signature page of the application needs to be received within 15 days of the electronic submission in order to keep the date of electronic submission as your original application date. Otherwise, the date of the application will be changed to the date your signature page is received in the DHHS office.
All printed applications and the signature page for electronically filed applications must be sent to:

Note: When you print your GAPS form, it will look different from the form that you filled in. It will print using a DHHS form with all your information included.
