Medicare Part D Prescription Drug Program

Effective July 1, 2010, the GAP Assistance Pharmacy Program for Seniors program will no longer be available due to lack of state funds. This program provided assistance with Medicare Part D drug coverage when enrollees reached the coverage gap (sometimes called the "donut hole").

The recent Affordable Care Act (Health Care Reform) provides assistance for prescription drug coverage for Medicare beneficiaries. This includes a one-time $250 rebate check when individuals reach the coverage gap. Gap Assistance Pharmacy Program for Seniors (GAPS)

The GAPS (Gap Assistance Pharmacy Program for Seniors) form is for SC residents only. It is for seniors 65 or older who are Medicare eligible and have a gross household income below 200 % of the Federal Poverty Level and who need help "filling the gap" with their Medicare prescription drug plan.

IMPORTANT!!! If you are a senior 65 or older, you can submit your GAPS Application online! You must register (on this site) to submit your GAPS application electronically.

All senior residents of South Carolina who are 65 or older may print the form and mail it to the SC Department of Health and Human Services for processing.

IMPORTANT: The system will time out after 30 minutes of inactivity. In order to save your application and be able to go back to it at a later time, you must Register or Login.

IMPORTANT! For electronically filed GAPS applications

The signature page of the application needs to be received within 15 days of the electronic submission in order to keep the date of electronic submission as your original application date. Otherwise, the date of the application will be changed to the date your signature page is received in the DHHS office.
